REQUIRED: Daily Screening Questions for COVID-19
It is REQUIRED that you ask and answer these questions DAILY before leaving home and before entering John Bapst. No student or employee should come to school if actively symptomatic. PRINT THIS FORM OUT DAILY, respond, and turn it in as you enter the foyer of the school.
REQUIRED: COVID-19 Sympton Check Form for School Visitors
Visiting the school for any reason? Please DOWNLOAD, PRINT, AND COMPLETE this form. All visitors must check-in at the main office where they will turn in their completed form.
REQUIRED: Parent/Guardian Informed Consent Form
John Bapst requires that a signed Parent/Guardian Informed Consent Form be on file for each enrolled student. Please fill out the consent acknowledgement form and send it with your student on the first day of school.
REQUIRED: Out-Of-State Travel Form
The Maine CDC requires a 10-day quarantine if you travel out of state. Students/staff who plan to travel outside of Maine must submit this form to the school.

For safe physical distancing, we’ve introduced one way traffic flow through our halls and up and down our stairs. Signs are posted throughout the school but here’s a map to help.

- 2020-2021 Key Dates
John Bapst is preparing to reopen according to schedule on August 25, 2020, the first scheduled class day for new students. On August 26 classes will begin for all students. The school’s Reopening Committee is working with professionals from the Northern Light Healthcare System to make sure the reopening is safe for students and staff.
